Updated Screening Questions January 2022
The following SCREENING questions are based on the recent updates from the Ministry of Health of Ontario and the current Covid-19 self-assessment tool.

SCREENING QUESTIONS
*All questions have a yes/no option unless otherwise outlined below.
1. Are you fully vaccinated against Covid-19? (Fully vaccinated is currently defined as two doses of any of Pfizer, Moderna or AstraZeneca/Covishield, or any combination of these, or one dose of Janssen/Johnson & Johnson, and you have received your final dose at least 14 days ago.)

Please note: your answer to question #1 is for screening purposes only and does not affect your ability to access physiotherapy services
2. Do you have any of the following symptoms? Please check all that apply.
Fever and/or chills
New onset of cough or worsening chronic cough
Shortness of breath
Decrease or loss of sense of taste or smell
Runny nose or nasal congestion (not related to allergies or other known cause)
Headache (new, unusual or long-lasting)
Extreme fatigue
Sore throat
Muscle aches or joint pain (unusual or long-lasting)
Gastrointestinal symptoms (such as vomiting or diarrhea)
3. In the last 10 days, has someone you live with been sick with symptoms associated with Covid-19 or tested positive for Covid-19 (either on a PCR or rapid test)?
4. In the last 10 days, have you tested positive on a rapid antigen test or home-based self-testing kit, or are you awaiting the results of a PCR test for Covid-19?
5. In the last 10 days, have you received a Covid Alert exposure notification on your cell-phone?
6. In the last 10 days, have you been identified as a "close contact" of someone who currently has Covid-19 (confirmed by PCR or rapid test)?
7. In the last 14 days, have you travelled outside of Canada? Please check all that apply.

8. Are you concerned you may have been exposed to a known or potential Covid-19 infection in any other setting (e.g. there is an outbreak at your workplace, school or other place you attend frequently)?
If you answered NO to all of the screening questions and have NO SYMPTOMS, I look forward to seeing you at your appointment today.
Please note, a medical mask that securely covers your nose and mouth will be required to be worn at all times while in the clinic. If you do not have one, I will be happy to provide you with one at the start of your appointment. If you have an N95 or KN95 mask on already, you do not need to switch.
If you answered YES to any of the screening questions, please contact me AS SOON AS POSSIBLE, to see whether you can attend your appointment or need to reschedule. I am currently not charging a cancellation fee for cancellations due to illness.
